No one would argue that, today, we live in highly uncertain and historically unprecedented times. The world-wide effects of the COVID-19 global pandemic have changed our lives in many ways. Amid the uncertainty of this evolving situation, we dropped our contracts with the insurance plans and closed Understanding Women, PC. Please refer to the side panel to request your records directly from Clary Document Services.
Dr. Sweeten will continue specializing in cash-pay hormone replacement therapy and aesthetic procedures primarily. If this situation changes, we will keep you abreast of our status. Please check our website www.drsweeten.com for updates and look for our new social media outlets.
It has been an honor to be your doctor the past 25 years. I have greatly valued our relationship and thank you for your loyalty and friendship over the years.

Clary Document Management will be the custodian of your records. Your medical records are confidential. A copy of your medical records can be made and sent only with your written permission. If you would like a copy of your records sent to you or sent to a physician, click here for an authorization form. The form requires an email address as a destination for the copy. Please print out the records authorization form, complete and sign it. CDM need to have an actual signature in order to release your records.
Send the signed form by mail or by fax to Clary Document Management - see address and fax number below. If your visit to my office took place before 2013, the records have been deleted.
